Pulled off to side of bike path (into tall grass) and my front wheel dropped into a chuck hole and I went over the handlebars. Add note to this report

Explanation:
I was worried about stopping on the path (this was the STP, and there were LOTS of riders). But I should have signaled "slowing" and reduced speed on the path to >5 mph and THEN pulled off.
