Description:
At 9:15am driver was travelling North on NW 24th Ave in blue Jetta California license plate [omitted]. Driver initiated a left-hand turn to 60th Street NW (Westbound). I was travelling South on NW 24th Ave in the bike lane, but driver didn’t stop until a collision was imminent. At that time driver hit their brakes, and I was able to swerve away from your car to the West side of the road, where I ran off the road to avoid colliding with a parked van. Transition over curb flipped bike and I tumbled across grass strip. At that time driver immediately stopped, checked if I was ok, and exchanged information with me. Add note to this report

Explanation:
Severe crash, lots of adrenaline. Really should have called police and gotten a report - if I AM injured, and if she's low integrity, I'll be out of luck. Effectively no signal or warning before left - SLOW DOWN at intersections, even if you have right of way!

Blue late model Jetta initiated left turn, violating bike right of way (left hook). Bike speed about 20mph. Extreme evasive maneuvers avoided collision, had to do second evasion to avoid parked van, ran off road and crashed.
