Location: 1900-1998 NW Dock Pl, Seattle, WA 98107, USA
Geometry: Vehicle turned into oncoming bike
Injury: Moderate
Description: I was stopped at the stop sign on Dock st., waiting to turn left onto Shilshole ave. The car was driving east on Shilshole, and was going to turn left onto dock street. The car began their turn, but drove completely through the oncoming (right) lane of dock street, where I was standing, hitting me head on. I rolled up the hood and windshield, and landed on the ground to the left of the car in the left lane.

Edit** The accident actually occurred at the intersection of Shilshole and 17th ave. NW, not NW Dock Pl. Full report
Location: 2494-2564 10th Ave E, Seattle, WA 98102, USA
Geometry: Hit a pothole
Injury: Serious
Description: I was riding north on 10th Ave E. It was around 9:30 pm. It was pretty well lit. There is a large section on 10th Ave E just before it crosses over 520 that has a cluster of deep potholes, cracks and ruptured pavement. As my back tire hit one of the potholes, my bike flipped upwards and I could hear my back tire hissing as my head and left side of my body slammed into the ground. It is a bit fuzzy after that. I was lucky that a number of cars had stopped and blocked the road so I didn't get run over. At least seven people stopped, collected my stuff, assisted and someone drove my bike to my apartment. Location: 1300-1398 3rd Ave, Seattle, WA 98101, USA
Geometry: Bike lost stability and crashed
Injury: None
Description: I was riding up 3rd Ave beginning my commute home after work - about 5:40 pm. I was riding in the center (left) lane heading north - doing fine - probably traveling at around 10 - 12 mph. All of a sudden my bike wheel just slid out from under me on the wide metal grid covering the bus tunnel. I had to unclick my shoe on my right foot. Fortunately my left foot was not clicked in, so I was able to land on it before I went completely down. My bike was nearly all the way down, but I was able to extricate my right foot and swing it over so I could stand up and get the bike upright. The really scary part was that there was a bus right behind me....fortunately far enough back that it could stop before running over me! There were two buses in the right lane as well (they may have moved forward by the time I crashed)so I couldn't really have avoided the grid even if I'd wanted to. I've ridden this route quite a few times before, and even though it's the designated bike route through town, I've always found it pretty scary. While the bus drivers are generally courteous, they are very large compared to us cyclists, and a "freak" accident like this could easily mean someone's death if the bus were closer or didn't see the person. Location: Burke Gilman Trail, Seattle, WA 98107, USA
Geometry: Trail conditions
Injury: Serious
Description: Clear, cold sunny morning (8 a.m.) I had crossed the S-shaped tracks ~45 min earlier westbound and was now heading home eastbound. Frost on the rubber mat or railroad tracks caused my bike to quickly slide out from under me. I went down on my left side and my head slammed against the pavement. There was nothing I could have done to avoid this crash. I have crossed this set of tracks for over 10+ years without incident. I can now attest, with many others, that in certain weather conditions, this track crossing is extremely hazardous and alternate routes should be considered. Full report

Location: 4502 14th Ave NW, Seattle, WA 98107, USA
Geometry: Bike lost stability and crashed
Injury: Moderate
Description: It was raining lightly on a Wednesday afternoon. I was traveling Westbound on the road that goes under Ballard bridge. There is an old railroad track. Local bikers have a name for this section because so many people wipe out on it. I was one of them.
My front tire slid into the groove of the track and I went down sprawling. A man from a business on the corner came out to check and see if I was OK. He said he'd seen dozens of people go down in that exact spot.

Perhaps it's time the DOT fill in this bike trap. Full report
Location: Burke Gilman Trail, Seattle, WA 98107, USA
Geometry: Bike lost stability and crashed
Injury: Moderate
Description: Early in the morning, it was sunny but below freezing. At this point on the Burke Gilman trail one must make a sharp turn to cross the train tracks. Black ice/frost had formed overnight on the rubber mat between the rail tracks (this rubber was apparently placed there to promote traction but is obviously not an optimal material). While turning, my wheels slid out from under me, and I fell hard on my shoulder, which 24 hours later is still very sore. My head hit the pavement pretty hard, too, so it's a good thing I was wearing a helmet. Full report

Description: Was riding fast (15mph) to stay ahead of traffic on narrow bridge overpass that is the current bike route to head over the east side of the Ballard bridge. Front tire got got in gap in bridge and I went down quickly into side rail. Luckily there was no other traffic behind me or it would have been very dangerous. Tore my front tire, bent out my water bottle cages and ripped up my clothing on shoulder and back. Full report
Location: 248-298 Florentia St, Seattle, WA 98109, USA
Geometry: Bike turned into vehicle path
Injury: Moderate
Description: I was traveling west-bound on Nickerson St and making a left-hand-turn with a green light onto Florentia St. Cars headed east-bound on Nickerson were stopped all the back through the intersection beyond Florentia, The drivers in the eastbound lanes had left a space for vehicles to turn left. I yielded before making my turn to make sure the cars were stopped. Then I pedaled through the intersection and was struck by a car traveling eastbound through the intersection on the shoulder. The driver claimed that there were three lanes on Nickerson St west of Florentia. When I went to examine the intersection after the crash, I could see that Nickerson west of Florentia is clearly marked for only 2 lanes, however drivers will often use the shoulder of the road west of Florentia as a separate lane. Location: 4300 11th Ave NW, Seattle, WA 98107, USA
Geometry: Vehicle came from side
Injury: Serious
Description: I was heading east on NW 45th. I stopped at intersection on NW 45th and 11th NW. Proceeded through intersection when clear (Burke Gilman trail begins on the opposite side, next to Fred Meyer parking lot). Car coming northbound on 11th NW, failed to stop at the stop sign. In discussion with driver of car, she indicated she didn't see the stop sign and she didn't see me. She hit me in the rear wheel of my bike knocking me down on the pavement. I had almost completely exited the intersection when I was struck. I recall seeing her car when I entered the intersection but it was some ways away from the intersection and I assumed she would stop at the stop sign. Furthermore there was a fair amount of traffic and I had waited my turn and was taking my turn to cross the intersection. Full report
Location: Eastlake Ave NE, Seattle, WA 98105, USA
Geometry: Bike crossed into vehicle path
Injury: Moderate
Description: My fault. Tried to take bike lane entering onto Eastlake north of University bridge, did not wait for double articulated bus to enter Roosevelt/Eastlake. I merged with bus and did not have enough room between curb and tail end of bus which pinched me between bus and curb. Scraped curb and bars scraped on bus, lost control and went down as bus passed and did not stop. Fairly slow crash, left shoulder roll but pinched left arm on road under bar or brake lever, causing hematoma and severe bruising which "flowered" over the next several days. Two great guys in SUV's stopped, made sure I was OK, which I was except for bruised arm and brain fade. What was I thinking? I'm fine and wiser. Full report

Injury: Serious
Description: Riding downhill in the middle of westbound traffic lane (at approx 20-25mph), I suddenly encountered jaywalking transient in middle of lane. With little time to respond, I attempted to avoid him but still clipped him. Next thing I remember I was being helped to the sidewalk by a good samaritan passerby female. Transient appeared inebriated and free of injury, and he confirmed that he was alright. I had all the symptoms of a concussion and got picked up by a friend who took my disoriented self to the ER to find out I luckily had no fractures. Full report
