Description: Commuting on my daily route from downtown heading to Burien. I briefly go on 96th getting ready to merge right onto Des Moines memorial Drive. In the middle of the apex (corner) there is a storm drain cover. You do not see this until you are right in the middle of your turn. It happened so fast I had no idea what I hit. Then on further looking I noticed the storm drain cover. If you go around it you get into traffic if you go to the inside of it you get into the merging of road pavement to sidewalk pavement which is also no good. The light was green and I just didn't see the cover. Full report

Description: It was 6:15am, perfect conditions with little traffic. I was northbound going down 24th Ave E which is a long hill off of Capitol Hill. I approached Boyer Ave E in the far right lane with a solid green light going 30+ mph. Without warning, a car in the left hand turn lane facing the opposite direction, pulled directly into my path as to make a left turn onto Boyer Avenue (eastbound). The car was so close I had no time to act evasively and hit the breaks hard and laid the bike down. I crashed hard into the ground, the fork of the bike snapped, but I did not hit the vehicle. Had I, someone else would be writing this. I think I came to a stop just barely short of hitting the car. I think the driver may have been distracted by a car in the left hand turning lane facing the same direction as me. No excuse for not seeing beyond that car onto what was happening in the rest of the roadway. That's my only thought as to why he didn't see me, but he was also an older fellow as well. I had a bright 300 lumen headlight and reflective material stripes on my jacket, and a little on my tights. Not enough I guess. I'm just happy to be alive. Full report

Description: South Bound work van passed me in the right lane with less than 2' of clearance at excessive speed. To gain additional clearance I departed the roadway, finding the adjacent shoulder very rough. Attempt to regain the roadway resulted in the rear wheel sliding west out from under the bike. Description: Cyclist was "bombing" eastbound down bridge at high speed. Failed to either look for cyclists using path to continue westbound up bridge or thought he could make it through intersection before us. Problem cyclist couldn't make it through intersection curb cut (on red light) and instead ran into me and rolled into the street. No one was hurt. His bike lost a piece of something, my brake lever was a little bent, but not bad. This is a problem area to begin with as many cyclists cut the intersection at high speed instead of using the path. Description: Riding my bicycle to work in a southeasterly direction on sidewalk along Nickerson Street. Due to high volume traffic in this area, I typically ride my bike on the sidewalk. As I approached the alleyway on the south side of the Fremont Bridge Self Storage building (319 Nickerson Street), I did not see any cars as I looked straight ahead. With no warning, I was hit (T-boned) by a car that was exiting (driving northeast) from the alleyway to Nickerson St. The car did not stop at the sidewalk before crossing sidewalk - clearly the driver proceeded before it was safe to do so. Bike and I ended up in Nickerson St.

Description: Bike traveling southeast on Ravenna Pl at ~10 mph bright flashing light yellow and fluorescent green reflective jacket and helmet. Car traveling northeast on 24 ave NE went through stop sign/rolled out into oncoming bike, likely looking other direction (to the driver's right) and struck bike head-on at slow speed. Front bike wheel mashed/bike unrideable, bruised shin but no serious injury. Parked cars on West side of Ravenna pl made for very bad visibility. Waited for police ~1 hour, when he (Off. Carullo) came he suggested not filing a report. When I insisted on making a report, he took our information but did not get out of his car and it was unclear whether he appreciated what happened in any detail. The driver was not given a ticket. Description: It was 8:38 AM on 5/27/09 and I was going about 20 mph Southbound down Meridian Avenue about 2 blocks North of Northgate Way. I was riding defensively as usual, but I let my mind slip and assumed I was dealing with predictable and watchful drivers ahead of me. Wrong.

I was riding down Meridian at a pretty good pace with cars passing me on the left. As usual, I ride on the right side of the road just next to the curb. Cars pass alongside me and I pass them on the right from time to time. There was a line of cars back to the driveway on the right here. There was one car though, some stupid family 4-door sedan, stopped as all the other cars emptied the intersection on a green light. My depth perception was a bit off, as it appeared she was just zoning out and was going to eventually continue straight with the rest of traffic.

There was no blinker and the car was positioned straight in the middle of the road. This made me assume she wasn’t going to turn right into the driveway. I had two choices. I could get right behind the car like any other vehicle would (relatively safe), or I could stay on the right and just cruise by (slightly more dangerous). I chose the latter and paid the price.

Like she just made up her mind spontaneously, she began turning right while flipping her blinker on. The second it illuminated, I knew I was done-for and commenced evasive maneuvers.

I steered sharply to the sidewalk to avoid the collision, we collided, and I was thrown over the hood and cleared the shadow of that small tree. It was a glorious Superman impression. I probably spent about 2 meters of flying time enjoying my last moments as a happy biker today.

She stopped in traffic and I had her pull into the parking space where she was headed. I did a MOFA-style self-assessment from head to toe right in front of her. Here’s what I assessed.

- road rash on left forearm

- road rash/bruise on left hip
- no head or neck injuries, did not hit my head
- I was in mild shock. It’s hard to say, but I was definitely out of my mind a bit.
- new $100 bike jacket ripped up
- both brake hoods scratched and bent inward from rolling the bike on the pavement
- rear wheel out-of-true slightly, but my right turn mitigated a front wheel taco
- front derailleur won’t shift down now, cable ripped out from impact on lever

She ran out and asked me if I was all right. I said I was checking things out and I’d get back to her. After quickly looking at myself, I determined that my problems were minimal. Although I do regret not demanding about $100 in cash to pay for my ruined 2-day-old bike jacket. Annoying. Full report

There was moderate traffic and my tire caught on one of the train tracks and I crashed. Thankfully the on coming car was able to stop in time. Description: I was heading South on 24th Ave and the driver was heading North. He took an abrupt left turn onto 58th (without signaling) and crossed into my lane. I hit the brakes but there was no way I could stop in time. With both wheels skidding, I slammed into the side of the fender of his car.

I flipped over the hood and landed on my back on the street. His fender was crushed and my bike was destroyed.

Description: I was heading southbound on Roosevelt way in the bicycle lane going 25 mph. The car was travelling at about the same speed in the left hand lane about 5 car lengths ahead of me. Without signaling, the driver moved across the right vehicle lane and into the bicycle lane, coming to a stop with the nose of the car in the curb-side parking space and the tail of the car protruding about a foot into the right vehicle lane, completely blocking the bicycle lane. The car was angled about 45 degrees to the direction of traffic.

I firmly applied by brakes and skid sideways into the car, making contact with the right rear quarter panel. I was thrown from my bike and slid across the car's trunk, landing on my back in the rightmost vehicle lane. My head make contact with the pavement, but my helmet an backpack prevented me from sustaining injuries from my contact with the road. My only injuries resulted from my initial contact with the vehicle. Full report

Description: I was heading southbound, other cyclist heading northbound. There is a raised bridge that goes over the train tracks. On the southbound side there is a blind corner, approx. 45 degrees, about 10-15 feet after the incline ends. As I came down the ramp I turned the corner and immediately collided with the other cyclist. Both of us were at low to moderate speed. While we had about three feet of total braking distance before colliding, her lower arm was broken where my handlebar contacted her arm. Description: Shortly before 6 p.m., the cyclist, Brian Fairbrother, was riding alone northbound on the west sidewalk near 1177 Fairview Ave N, according to the traffic investigation report.

At this location, the facility splits for pedestrians and cyclists. The sidewalk has an unmarked curb cut, where cyclists are supposed to move to street level to cross the bridge on Fairview. Pedestrians may continue down the steps to cross a bridge at the water level.

There were no witnesses, but the investigation shows that Mr. Fairbrother missed the curb cut and crashed down the stairway near the south side of the bridge, landing face first on the sidewalk. He died as a result of his injuries. Full report

An oncoming rider was coming down the hill through the corners very fast, and paying attention to the construction not other riders/traffic. I had been on the right side to the lane, against the shrubs, but had no where to go.
