Location: 599-699 West Lake Sammamish Pkwy NE, Bellevue, WA 98008, USA
Geometry: Bike lost stability and crashed
Injury: Serious
Description: Due to the concussion I only remember bits. I was riding down West Lake Sammamish and noticed a landscaping truck parked in the bike lane. I slowed down and next thing I knew I landed hard on the pavement and took most of the impact on my chin. From here things get very fuzzy. I was taken to Overlake via ambulance and thankfully only have a concussion, road rash, a gashed up chin and one hell of a head ache. Could have been a lot worse. Thankfully I had great witnesses who took amazing care of me till medics arrived. Full report
Location: 599-699 West Lake Sammamish Pkwy NE, Bellevue, WA 98008, USA
Geometry: Bike lost stability and crashed
Injury: Moderate
Description: Coming down West Lake Sam @ about 23mph on broad bicycle shoulder. Started coming around semi-blind turn only to find a landscaping truck and trailer completely blocking the bicycle lane. Looked behind me to see if I could swerve into traffic lane to avoid it or if had to clutch both brakes. Didn't see a car, started to swerve into traffic lane and front wheel caught the road "lip" between the arterial and bike lane (not smooth transition). Basically lost control of bike at that point.

While waiting for ride home saw another cyclist go down in the exact same spot, only much harder (landed on her chin). Landscape truck immediately left after that, although I'd gotten details from them before that (license, phone, etc.) Full report

Location: Interstate 90 Trial Walk, Bellevue, WA 98008, USA
Geometry: Bike ran into object
Injury: Moderate
Description: When leaving the Sunset Elementary School grounds, the Interstate 90 Trail Walk begins to descend down a slight hill.

The area is mossy and covered by trees - and slick.

There is a sign right at the top of the hill, but by the time I saw the sign, it was too late.

I was not traveling more than 10 mph when I started down the slope, but it was like trying to stop on ice, and I crashed into the rock wall. Full report

Location: 10000-10198 NE 1st St, Bellevue, WA 98004, USA
Geometry: Bike ran into object
Injury: Moderate
Description: While riding my bike home tonight, I hit an improperly fitted manhole cover on westbound NE 1st St and 102nd Ave NE in downtown Bellevue. The manhole cover was raised enough (see photo below) that it instantly punctured both my inner tubes, ripped holes in my tires and, it turns out, did irreparable damage to my wheels. As soon as I hit it, I lost control of the bike and ended up skidding along the ground on my side. Luckily I was not actually hurt beyond a few bruises and a hole in my glove. More luckily there were no vehicles near me when it happened otherwise I could have been crushed. One motorist was kind enough to stop and check if I was okay. Full report

Location: 798 140th Ave NE, Bellevue, WA 98005, USA
Geometry: Vehicle turned into oncoming bike
Injury: Serious
Description: I was riding north on 140th, traveling at a speed of 25 mph. As I was passing through the intersection at 140th and 8th, the car stopped in the left turn lane from the opposite direction hit the gas to make a left turn. She either did not see me or misjudged my speed. She hit me head on. My bike was ripped into two pieces along the frame, and my helmet was split in half. I knocked the windshield in about a foot (according to the EMTs) and landed on the North side of the car. From what gather from talking to the EMTs and police the police report, and witnesses, my bike was hit by the nose of the car, my arm went into the windshield my head - with a helmet covering it - hit the crossbar of the windshield and then continued over the car where I landed in the street. Lucky for me it was a car and not and SUV. The driver was 16 years old and driving without a license. Full report

Location: 5201-5699 Hazelwood Ln SE, Bellevue, WA 98006, USA
Geometry: Bike swerved into other bike
Injury: Moderate
Description: I was headed uphill, northbound, and a car pulled completely into the opposite lane to pass me. It wasn't necessary and it was on a blind corner. As we all rounded the corner 2 bicyclists were coming down the hill (southbound) and suddenly found themselves headed straight for the car. They bumped into each other and crashed into the ditch. The car slowed down and the driver looked over at them, then drove off. They were OK, just scraped up. They didn't want to call the police. I think this could have been avoided if there were bike lanes on each side of this road. Since it's on the south Lake WA route and heavily used by bikes, bike lanes are certainly a no-brainer here. There is NO shoulder whatsoever. Full report
